What is a health risk assessment?

A health risk assessment (HRA) is a state-of-the-art questionnaire that can help determine your personal stage of health. Our HRA, the Healthy Life Survey, also includes advice on how to improve your overall wellness.

How does the Healthy Life Survey work?

Simply complete the survey, which takes about 15 minutes, and you’ll receive personalized feedback along with suggestions on how to improve.

Why is the Healthy Life Survey important?

Individuals who know their current health status and where they need to improve are much more likely to have success on the path to wellness.

Can I take the Healthy Life Survey more than once?

Yes. It is important to take the survey annually to see how your health habits have improved. However, once you take the survey, you will not be able to complete it again until 12 months have passed. You do have the option to take mini assessments within the health information/health coaching portal to assess your health status throughout the year.

Is my information kept private?

Your information is extremely secure and is not shared with anyone outside The Other Health Plan (TOHP) or our service partner, PATH. PATH and our member website are HIPAA (Health Information Privacy & Accountability Act) regulated and strictly compliant. Our partner relationships are exactly like the relationship your health insurance carrier has with pharmacists, dental plan carriers, and/or vision plan providers. These services are described as “third party administrators” or TPAs. All TPAs we work with are required to sign a Business Associates Agreement with PATH. We hold each of these TPAs to strict privacy standards.

Who views my information?

Only TOHP health professionals view your information. This includes our health promotion staff, our partners at PATH, and our lifestyle management health coaches who are HIPAA-compliant health professionals. No one else may see your information without your written consent.

Will my employer see my results?

No. Your employer will know only that some employees in your organization have participated and in some cases may know the names of employees who have completed the Healthy Life Survey (for incentive purposes only). No identifying health information will be shared with your employer without your written consent.

Can my results or information be used to deny me health benefits?

Absolutely not! Neither your employer nor your health plan can use wellness program data to change your access to benefits or change the quality of your benefits. HIPAA exists, in part, to end such behavior by employers or health plans.

What do you do with the Healthy Life Survey results?

First, we provide you with a personalized report and a set of recommendations specific to your personal health status.

Second, we compile group reports on the data for your employer. These reports are anonymous and show aggregate data only. They allow us to show cause for important wellness services in your area. For example, if 80 percent of the people in your organization say they aren’t eating enough fruits and veggies, we’ll help your employer establish a campaign to improve these statistics.

Where is my personal information stored?

Our database is stored on a dedicated server owned by our service partner and is maintained by a HIPAA-qualified hosting team. No one else has access to the database and the security standards meet HIPAA requirements. Our web development and hosting team has signed a privacy agreement that matches the agreement signed by our other partners.

Is the information I provide sold to partners?

HIPAA does not allow us to sell your protected, personal health information (PHI). PHI includes anything that can identify you in any way, including your address, phone number, name, email, etc. Our partners are obligated to the same standards and cannot use your PHI for any reason other than to serve you within the bounds of this program.
